This is a comment on PEPFAR program expenditures.
For several years, PEPFAR has sponsored programs to provide male circumcision in Africa in an effort to reduce the female to male transmission of HIV infection.
This program is based on evidence from three randomized clinical trials (RCTs) that were carried out in Africa. These three RCTs have now been shown to be hopelessly flawed and all evidence from them is unreliable. The incidence of HIV infection in many African nations is higher among circumcised men as compared with intact men. Furthermore, male circumcision has been demonstrated to cause a sharp increase in male to female transmission of HIV.
The male circumcision program, therefore, is likely to worsen the epidemic and should be discontinued.
